Claim adjustment reason code · Contractual obligation

CO-5
Procedure code or bill type inconsistent with the place of service

What CO-5 means

The service is not valid in the place of service reported on the claim.

The CO group code means this amount is a contractual obligation: under most contracts you cannot bill it to the patient.

Common causes

  • Wrong place-of-service code, for example telehealth or office vs facility

How to fix it

  1. Correct the place of service and resubmit
  2. Check telehealth POS rules for the payer

Check the remark code (RARC) that came with the denial and the payer's own policy, since rules vary by payer and plan. This is a plain-English summary written by GetMax; the official code text is published by X12 at x12.org/codes.
