Public Provider API, v1.0 launch

- NewPublic Developer API shipped. Three endpoints, list, get, filters, scoped to your practice via the x-api-key header.
- NewDeveloper dashboard for key management: create, name, view prefix, and revoke. Up to 10 active keys per practice.
- NewSingle scope at launch: providers:read. Additional scopes will be added as new endpoints open.
- NewSigned S3 URLs for provider profile images, returned fresh on every request.
- SecurityKeys are stored only as SHA-256 hashes. Raw value shown once on creation.
