In-house billing vs outsourced RCM: how to decide

GetMax RCM team7 min read

The honest trade-offs between running medical billing in-house and outsourcing revenue cycle management, plus the hybrid option most practices overlook.

Every growing practice reaches the same question: keep billing in-house, or hand it to an outside team? Both can work. Both can fail. The right answer depends less on cost per claim than on whether someone will own the whole revenue cycle and be accountable for the results.

The case for in-house billing

  • Control. Your staff know your providers, your patients and your quirks.
  • Speed of communication. A question about a charge is a walk down the hall.
  • Patient experience. Billing questions are answered by people who know the patient.

The risks:

  • Key-person risk. When your biller is on leave or quits, billing stops.
  • Coverage gaps. One or two people rarely have time for denials, AR follow-up and credentialing on top of daily claims.
  • Hidden costs. Salaries, benefits, training, software, clearinghouse fees and turnover add up.

The case for outsourcing

  • Capacity. A team can work denials and old AR that an in-house person never reaches.
  • Expertise. A billing company sees many payers and specialties and knows their rules.
  • Continuity. Work does not stop when one person is out.

The risks:

  • The black box. Many practices that outsource cannot see what is happening to their claims until the monthly report.
  • Misaligned priorities. If the company is paid a percentage of collections, small or hard claims may not get attention.
  • Lock-in. Your data and history sit in the vendor's systems.

Questions to ask before you decide

  1. Do we know our net collection rate, denial rate and days in AR today?
  2. Is anyone working our denials and AR over 90 days every week?
  3. What happens to billing if our main biller leaves next month?
  4. If we outsource, will we be able to see every claim and every note?
  5. Who owns our data and how do we get it back?

The hybrid option

It is not all or nothing. Many practices keep the front desk and patient billing in-house and hand off the work that needs volume and expertise: denials, AR follow-up, credentialing or old AR cleanup. The key is that everyone works in the same system, so nothing is hidden.

That is how GetMax works. You can run your revenue cycle yourself on the GetMax platform, have our RCM team run it, or split the work. Either way your team sees every claim live. Want a number for your own practice? Try the RCM cost calculator.
